How to Join a Gene Therapy Clinical Trial
For many families living with a rare genetic disease, a clinical trial is the only route to treatment. From the outside the process can look closed off. This guide explains how to find a gene therapy trial, how to read its eligibility criteria, and which questions to ask.
What the trial phases mean
Phases are the steps in a therapy's research journey. Each one answers a different question:
- Phase 1: Safety and dose. Usually a small number of participants. The goal is to see whether the therapy is tolerated.
- Phase 2: First signals of efficacy plus the side-effect profile. More participants.
- Phase 3: Comparison against current standard care. This is the core evidence for an approval application.
- Phase 4: Long-term follow-up after approval.
- Early Phase 1: First-in-human, very small studies.
A large share of gene therapy trials sit at Phase 1/2. That means the therapy is not yet proven — but research access may be possible.
What the trial status tells you
- Recruiting — open to new participants.
- Enrolling by invitation — open only to a pre-identified group.
- Active, not recruiting — ongoing, but not accepting new participants.
- Completed — finished; results may be published.
- Terminated / Withdrawn — stopped or never started; the record states why.
How to read eligibility criteria
Every trial record lists inclusion and exclusion criteria under "eligibility criteria". In gene therapy trials, the most common deciding factors are:
- A confirmed genetic diagnosis — most trials require laboratory documentation of a pathogenic variant in a specific gene.
- Age range — often narrow, especially in childhood-onset conditions.
- Disease stage or a specific functional score.
- Prior treatments — some trials exclude anyone who has already received a gene therapy.
- Pre-existing antibodies to the AAV vector — a frequent exclusion in AAV-based trials.
The process, step by step
- Establish your genetic diagnosis precisely: which gene, which variant, which laboratory report. Every application rests on this.
- Review the active trials on the relevant gene page and note the NCT number.
- Open the ClinicalTrials.gov record and find the site and contact details under "Contacts and Locations".
- Talk to your treating physician — applications usually proceed via clinical referral and require your medical records.
- Contact the site; pre-screening is normally done by phone or email.
- During informed consent, ask about risks, the length of follow-up, and the travel and cost burden.
Questions worth asking
- Does this trial have a placebo arm? If so, what is my chance of receiving placebo?
- How many visits, and over how many years?
- Who covers travel, accommodation and testing costs?
- Will I still have access to the therapy after the trial ends?
- What serious side effects are known, and how are they monitored?
- If I receive this therapy, could it prevent me joining another gene therapy trial later?
Joining from outside the trial country
Gene therapy trial sites are concentrated in the United States and Western Europe. Joining a trial abroad is possible, but travel, visas, long follow-up schedules and cost are real barriers. Some trials cover costs for participants travelling from other countries — always ask.
